Program Documents
The agency requires a Proof of Work Order form be completed to reserve program funds for each installation, as well as submission of a signed Certificate of Destruction form ensuring the old stove or insert was rendered inoperable and will be surrendered to a recycler to be used only as scrap, before a retailer or contractor is reimbursed for a redeemed coupon.
Additionally, a Recycling Report is due within 30 days of submitting the Certificate of Destruction, to verify the old device has been properly scarpped.
These forms are included in the packet sent to participating retailers; downloadable copies are also available here.
